New Student Registration

All students entering the Salem Central School District can register by following the instructions below or by contacting Laura LeBlanc in the school's counseling office:

New Student Registrations:

1. Pre-Register! Click the "Continue" button to the left and fill out the Pre-Registration form.

2. Please print and complete a registration packet for each new student. If you're unable to print it yourself, reach out to Mrs. LeBlanc to arrange a mailed version or to pick it up from the school.

3. Submit registration paperwork along with the following documentation:

__ Birth Certificate.

__Official immunization record and current physical.

__ Custody Paperwork, if applicable. If the student is not a biological child, documents must be provided which prove a transfer of custody.

__ Proof of Residence (Options include: a lease, utility bill, a license or other ID, a pay stub, tax forms, custody or guardianship papers, voter registration documents, etc.).

* If you’re unable to provide any of the above proofs of residence, please reach out for an Affadavit of Shared Residence/Affadavit of Residence Landlord/Owner Statement. This form must be notarized.*

Once you've filled out the paperwork and collected your documentation, please reach out to Mrs. LeBlanc to arrange a time to dropoff! Also feel free to mail paperwork in or scan to laleblanc@salemcsd.org.
